Guidelines:
- State what you want to learn.

Example:

Purpose: Identify who the survey is intended for.
Sections to include:
- Audience Type: e.g., learners, trainers, partners, community members
- Demographics: e.g., age, gender, education, region
- Sample Size Goal: e.g., 100 responses minimum for statistical relevance

Purpose: Outline how the survey will be administered.
Include:
- Survey Mode: Online (SayPro platform, Google Forms), In-person, Phone
- Distribution Channels: Email, WhatsApp, Social media, SMS
- Timeline: Survey start and end date
- Frequency: One-time, Quarterly, Annual, Post-event
Example:
