SayPro Documentation

Objective: Ensure all findings from the physical barrier assessment are formally documented for submission to SayPro. This includes complete lists, reports, visual evidence, and improvement recommendations.


1. Required Submission Components

All SayPro teams and contributors must compile and submit the following documents:

Barrier Identification Lists

  • Lists of up to 100 physical barriers generated from GPT-powered prompts.
  • Must be organized by:
    • Facility Area (e.g., entrance, restroom, parking)
    • Type of Barrier (e.g., structural, sensory, mobility-related)
    • Population Affected (e.g., elderly, disabled, parents with strollers)

Format: Excel spreadsheet or SayPro List Template (.csv or .xlsx)


Detailed Site Reports

  • On-the-ground or virtual assessments conducted using SayPro tools.
  • Must include:
    • Facility name and location
    • Photos (if applicable)
    • Notes on severity, frequency, and urgency of each barrier
    • Observations from staff or patient feedback
    • Dates and names of assessors

Format: SayPro Report Template (.docx or PDF)


Suggestions for Improvement

  • Actionable recommendations tied to each identified barrier.
  • Should consider:
    • Cost-effective or short-term solutions
    • Long-term infrastructure upgrades
    • ADA or local accessibility standards
    • User-centric design improvements

Format: Included in report or as a separate Recommendations Sheet


Summary Cover Sheet

  • Brief summary of findings
  • Facility name, assessment team, and contact information
  • Number of barriers identified
  • Number of suggested actions provided

Format: SayPro Cover Sheet Template (auto-generated via platform)


2. Submission Process

  • Upload all documentation via the SayPro Monthly Submission Portal
  • Ensure files are named using the following format:
    FacilityName_MonthYear_DocumentType (e.g., GreenClinic_June2025_BarrierList.xlsx)
  • Deadline: June 30, 2025, 23:59

3. Post-Submission Review

  • SayPro Quality & Accessibility Teams will review submissions in July.
  • Feedback will be issued by July 15.
  • Top submissions may be selected for feature in SayPro Best Practice Highlights – Q3.
